How to merge patients

If there are duplicate patient records for the same patient, you can merge these patient records together. Please note that not all users may have this functionality as your Trust or Organisation will choose who should have the ability to do this e.g. just MHAA, or AMHPs. Please contact your MHA Office or your team lead if you have any questions regarding this.

 

How to merge patients

One patient is named Patient A, the other named Patient B. You need to merge Patient B into Patient A.

  1. Open the patient page of the patient you would like to merge the patient into

  2. Click ‘Action‘ on the patient banner for Patient B.

  3. Click ‘Merge Patient‘

    1. You will see a warning banner

  4. ‘Please choose your reasoning for merging the patient.

  5. Input the details of the patient you want to merge the record into (you only need to input the patient’s surname)

  6. Select the patient

  7. Click ‘Merge’

  8. Click ‘confirm’

 

What happens to forms once you have merged patients?

Draft forms:

  • The patient information for draft forms will update automatically to reflect the new patient.

Signed forms:

  • Patient information will not be updated on any signed forms or form part.

How to undo a merge

You have 30 days to undo the merge. All forms for Patient B will be restored back to Patient B.

Any new forms created for Patient A can be restored back to Patient A or B. You can pick the forms you want to restore when you undo the merge.

 

  1. Click ‘Action‘ on the patient banner.

  2. Click ‘Unmerge Patient‘
