How to search for a patient

You can search for a patient to either start a form or see any in progress or completed forms for that patient. You can search for all patient’s within Mersey Care that have had a form created on Thalamos.

1. Go to search patient.

2. Type in the known patient information. You only need to enter the patient’s surname as a minimum to return a match. Click ‘Search’.

3. If the patient information you have searched for has multiple matches you will be shown all matched and be shown a warning banner. You will need to select the correct patient and and click ‘Continue’.

4. If this is not the correct patient or you would like to search again you can click ‘Clear Search' and perform a new search.

6. Once you have selected the correct patient you can start a form. You can also see any in progress forms on Thalamos or any form previously completed for the patient on Thalamos by someone in your organisation.
