How to receive section papers and complete an H3 on Thalamos

  1. You will receive an email notification when forms have been sent to your team.

  2. Click ‘view and action MHA form’ and log in, to view the form.

  3. To view the form, you can go to your caseload, search the patient or go to notifications.

    • Ensure your profile is up to date and shows your accreditations.

  4. If you have both medical recommendations, and the application you can begin your H3 form. If you need another or new form, click ‘request form’

  5. Click ‘start form,’ next to Record of Detention

  6. Complete the form and click review.

  7. Click preview pdf and sign when completed to add your signature.

  8. Send to the MHL Team in hours and the ward team out of hours. You can send these forms to anyone, even if they do not have an account.

  9. You can then review the form and its progress in the activity log which will be available in your caseload.

How to complete an H3 without section papers on Thalamos

  1. If you do not have necessary section papers, you can search/create the patient to request any forms to be completed.

  2. Click ‘start form’ next to the admission forms

  3. Click ‘request form’ to request a form

    • You can request these forms from anyone, even if they do not have a Thalamos account- just input their email address and they will receive a link.

  4. If you have received all appropriate documentation, you can click ‘start form,’ next to the H3 form

  5. Complete the form and click review.

  6. Click preview pdf and sign when completed to add your signature.

  7. Send to the MHL Team in hours and the ward team out of hours. You can send these forms to anyone, even if they do not have an account.

  8. You can then review the form and its progress in the activity log which will be available in your caseload.
