  1. Before you begin a from you must establish who the patient if that you are creating the form for. There are two ways to do this, you can either search for a patient or go to the patient from your caseload. Please see our help guide on how to do this here.

  2. Once you are on the patient, you can select the type of form you want to complete. For an medical recommendation, click the start form button next to Admission forms.

  3. You may be able to see some admissions already in progress for the patient, these are forms which have been started by someone in your team.

  4. You then have the option to ‘start’ the form, or choose ‘no recommendation.’ If you choose ‘no recommendation,’ you will be required to enter a reason why you have not chosen to make a medical recommendation for the patient.

  5. Complete the form and click review.

  6. Click sign when completed and add your signature. Ensure all the information is correct before signing.

  7. Once signed you can send the form. You can send the form to anyone, as long as they have an approved email address (e.g. nhs.net, or .gov.uk) regardless of if they have a Thalamos account.

Q: Can I complete and send a joint medical recommendation?

A: Yes, you can select a joint medical recommendation from the drop down list. You can then send the form to the next doctor to complete regardless of if they have a Thalamos account or not. If they do not have a Thalamos account you can enter their email address (must be an approved domain and not gmail or hotmail) and they will receive an guest log in to view and complete a form.
