You have accidentally created a new patient on Thalamos, with an already existing patient page.

One patient is named Patient A, the other named Patient B. Patient B was created in error. Open the patient page of the patient you would like to merge into the other (Patient B to be merged into A).

  1. Click ‘Action‘ on the patient banner for Patient B.

  2. Click ‘Merge Patient‘

  3. You will see a warning banner

  4. ‘Please choose your reasoning for merging the patient.

  5. Select the patient

  6. Click ‘Merge’

  7. Click ‘confirm’

All forms created for Patient B will now be in Patient A’s patient page.

Any in-progress forms will be updated with the correct patient information.

What happens to forms once you have merged patients?

In progress forms:

The patient information within in-progress forms will update automatically to reflect the new patient

Signed forms:

These forms will remain not be updated, however they will show a warning banner ‘patient detailed edited‘.

How to undo a merge

You have 30 days to undo the merge. All forms which were Patient B’s before the merge will return to Patient B when you merge.

You will have the choice to move any forms created for the merged patient between the two once merged.

  1. Click ‘Action‘ on the patient banner.

  2. Click ‘Unmerge Patient‘
