You can search for a patient to either start a form or see any inprogress in progress or completed forms for that patient.

The local patient index is a record of patient indexes maintained in the ‘data sharing lead’ organisation established at initial programme stages. If a record is found, then this will be returned to the user to confirm if the correct person is identified. If no record if found via PDS or in the local patient index, a new patient index can be created.  

You can search for all patient’s within Mersey Care that have had a form created on Thalamos.The patient context provides an oversight of old forms and forms in-progress for the person who is being assessed. Patient history can be accessed by users in the organisation to inform resources allocation and strategic planning. Patient data is displayed in real time to prevents lost forms or unknown MHA interactions and to improve MHA administration. The patient context is established at point of assessment and is auto populated onto the form that is being completed to reduce errors and administrative burdens on frontline teams. 

  1. Go to search patient.

  2. Type in the known patient information. You only need to enter the patient’s surname as a minimum to return a match. Click ‘Search’.

  3. If the patient information you have searched for has multiple matches you will be shown all matched and be shown a warning banner. You will need to select the correct patient and and click Continue’.

  4. If this is not the correct patient or you would like to search again you can click ‘Clear Search' and perform a new search.

  6. Once you have selected the correct patient you can start a form. You can also see any in progress forms on Thalamos or any form previously completed for the patient on Thalamos by someone in your organisation.
