What forms can I use on Thalamos?

Phase 1 of the roll out will focus on in-patient units. Your trust will be going live with the following forms:

In-hospital: H1, H2, H5, H6

Treatment: T2

Discharge: s23, Discharge from holding powers

Admission and CTO forms will go live in phase 2 of the rollout as we engage with AMHP and community teams.

How do I log in to Thalamos?

You should have a Thalamos desktop Icon which you can click to open up the Thalamos log in page. If you cannot see this,you can go to emha.thalamos.co.uk (for MerseyCare users), or https://emha.thalamos.co.uk/login/nscht (for NSCHT users). See our help guides on how to save this as a desktop shot cut here.

Log in by clicking ‘Continue with ‘your Trust’. You do not need to enter a email and password. If you have having difficulties logging in please contact Thalamos at support@thalamos.co.uk or call 0203 886 0385.

What should I do if I do not have access to the correct team?

I’ve made a mistake, can I edit a form?

If a form needs to be rectified under s15 of the Act, we advise you to print the PDF and make the amend by hand as you normally would do with a paper form.  

If a form needs to be redone, we advise you to create a new form and complete it again. 

We are working on digitising the amend function to ensure a form can be amended digitally. This feature will allow users to redo a form with previous data populated and/or to amend a form under s15.

I have been sent a form with a mistake, can I send it back?

Once you have been sent a form, you will have the option to ‘Accept’ or ‘Decline’ it. If there is a mistake on the form you can click decline, and input the reason why. This will then be sent back, and the person will be notified why.
