If you receive applications or medical recommendations with errors, you can now legally request these forms to be amended under Section 15 of the Act on Thalamos. The request can only be sent if the H3 has been signed within 14 days after admission.

If the medical recommendation or the application has been done on paper, we advise you request the signee to make the amend by hand as you normally would do with a paper form. If a form needs to be redone, we advise you to create a new form and completed it again.

How to send a request

  1. Click menu on the form that needs to be amended under section 15, and click request amend.

    • The request amend button will only show during the 14 days.

  1. Select a reason from the list provided

    • You can choose as many reasons as appropriate, depending on the error which needs to be amended.

  1. Enter additional information for this request (aka, patient name incorrect, it should be John Doe, not Joan Doe) and click 'confirm.

  1. The summary will change to amend requested, and this request will now be visible in the activity log

The signee will receive an email and in-app notification, letting them know that there is a form which needs to be amended and your reasoning for this request. If they choose to amend the form, it will be send it back to the MHA team.

  1. You will have received an email and in-app notification that the amend has been completed

  2. You will be able to view the ‘Old version’ with the error, and the amended ‘current version’ without the error.

  1. The summary will now show as ‘amend completed’ and once you have scrutinised the forms, you can mark the administrative scrutiny as ‘complete.’

Once you have marked the forms as scrutinised, you will no longer be able to request an amend.
