  1. Before you begin a from you must establish who the patient if that you are creating the form for. There are two ways to do this, you can either search for a patient or go to the patient from your caseload. Please see our help guide on how to do this here.

  2. Once you are on the patient, you can select the type of form you want to complete. For an H5 form, click the start form button next to Hospital Forms.

  3. If you will be completing the form, click ‘start new form’ and start typing H5 in to the select form box.

  4. You will then be able to click H5, and be able to click the ‘start form’ button.

  5. If there is already a H5 form in progress for this patient, you will be asked if you would like to start a new one or continue the current form.

  6. Complete the form as you would complete the paper form, by either selecting the correct option or entering text into the boxes.

  7. Once you have completed the form you can click ‘review’. You will then be able to review the form in the pink MHA format. If you notice any mistakes, press ‘go backto edit the form.

  8. If you are happy with the form, click sign. You can then enter your saved signature, draw your signature or upload one. Confirm that the signature will be the electronic representation of your signature by checking the box and then click 'confirm'.

  9. You can then enter the name of the person or team you would like to send the form to and you can select it from the dropdown list.

  10. You can then click send.

  11. If you have been sent an H5 form to complete, you will receive an email notification and a notification on Thalamos. The form will also appear in the caseload.

  12. Select the form, and click continue. You can then complete part 2 of the form as you would complete the paper form, by either selecting the correct option or entering text into the boxes.

  13. Once you have completed the form you can click ‘review’. You will then be able to review the form in the pink MHA format. If you notice any mistakes, press ‘go backto edit the form.

  14. If you are happy with the form, click sign. You can then enter your saved signature, draw your signature or upload one. Confirm that the signature will be the electronic representation of your signature by checking the box and then click 'confirm'.

  15. The form will then be automatically sent back to the person who completed part 1. They will be notified, and they can then complete part 3 of the form.

  16. Once part 3 of the form is completed, you can then enter the name of the person or team you would like to send the form to and you can select it from the dropdown list.

  17. You can then click send.

  18. The form will appear in your caseload and the team caseload until legal scrutiny has been marked as complete. It will then more to the archive.
