A Mental Health Act Administrator has requested an you to amend for a medical recommendation or an application you have signed.

If the form was completed on paper, we advise you to amend the form by hand as you normally would do with a paper form. If a form needs to be redone, we advise you to create a new form and complete it again.

How to amend a form

  1. You will receive an email and in-app notification letting you know that an amend has been requested by the MHAa. It will also show in your caseload as an in-progress the MHA Team have requested you to amend a form.

    • You will be able to see the reasoning they have requested the amendaccept or decline the request

  2. You will be able to amend the form once you have accepted the option to accept or decline the request for an amend.

  3. The summary will now show ‘amend in progress’

  4. Once you accept the request, click ‘menu' and ‘start’ on the in progress form,

  1. . Please make sure you only change the information that was requested to be amended. The reason will be displayed at the top of the page.

  • Only change the information that you have been requested to change and click ‘review.’

    1. One the change has been made, you will have to confirm the amend.

      • Click review once you have amended the form

    1. Click confirm once you have reviewed the form.

      • The form will automatically be sent back to the MHA

    administrator
      • legal team once you confirm.

    1. The MHAa MHA Team will have received an email and in-app notification that once the amend has been completed.

    The MHAa will have access to both the 'Old version’ with the error, and the amended ‘current version’ without the error
    1. You will be able to view both version as displayed below.

      • The original form version is marked ‘Old version’ and the amended form version is marked ‘current version’.

    The MHAa can then mark the form as scrutinised and the form will then move into your ‘archive’
