  1. Open the patient page of the patient you would like to merge the patient into

  2. Click ‘Action‘ on the patient banner for Patient B.

  3. Click ‘Merge Patient‘

    1. You will see a warning banner

  4. ‘Please choose your reasoning for merging the patient.

  5. Input the details of the patient you want to merge the record into (you only need to input the patient’s surname)

  6. Select the patient

  7. Click ‘Merge’

  8. Click ‘confirm’

Any in-progress forms will be updated with the correct patient information.

What happens to forms once you have merged patients?

How to undo a merge

You have 30 days to undo the merge. All forms which were Patient B’s before the merge will return for Patient B will be restored back to Patient B when you merge.

You will have the choice to move any Any new forms created for the merged patient between the two once mergedPatient A can be restored back to Patient A or B. You can pick the forms you want to restore when you undo the merge.

  1. Click ‘Action‘ on the patient banner.

  2. Click ‘Unmerge Patient‘
