You can add a file note to any form which has been sent to you. Only MHA Admins will be able to add a file note to forms.

  1. Choose the form you would like to add a file note to and click ‘menu’

  2. Click ‘add file note’

  3. Enter any notes you would like to be included

    • These notes will be saved as a page attachment to the PDF

  4. Click confirm, once you have completed your notes.
