If you receive applications or medical recommendations with errors, you can now legally amend these forms digitally, on Thalamos, as long as there is an H3 present.

If the H3 has been done on paper, you will need to revert back to your previous processes.

  1. Once you have accepted the forms, you can choose the form you would like to request an amend for by clicking ‘menu,’ and ‘request amend.’

    • The amend can only be completed by the person who signed the form.

  1. You will need to choose a reason from the list provided

    • this list will look different for an application and medical recommendation

  2. You can choose as many reasons as appropriate, depending on the error which needs to be amended.

  1. Enter additional information for this request (aka, patient name incorrect, it should be John Doe, not Joan Doe) and click 'confirm.

  1. The summary will change to amend requested, and this request will now be visible in the activity log for the form

The signee will receive an email and in-app notification, letting them know that there is a form which needs to be amended and your reasoning for this request. If they choose to amend the form, it will be send it back to the MHA team.

  1. You will have received an email and in app notification that the amend has been completed

  2. You will be able to view the unamended ‘Old version’ with the error, and the amended ‘current version’ without the error.

  1. The summary will now show AS ‘amend completed’ and once you have scrutinised the forms, you can mark the administrative scrutiny as ‘complete.’

Once you have marked the forms as scrutinised, you will no longer be able to request an amend.
