Before you start a form you first need to establish which patient you are creating the form for. You can search for a patient if someone in Mersey Care has already created a form for them on Thalamos. If you need to start a form for a patient that is not known to your organisation new patient or one which no one has created a form for yet, you can ‘create a patient’.
