You can search for a patient to either start a form or see any inprogress or completed forms for that patient.
The local patient index is a record of patient indexes maintained in the ‘data sharing lead’ organisation established at initial programme stages. If a record is found, then this will be returned to the user to confirm if the correct person is identified. If no record if found via PDS or in the local patient index, a new patient index can be created.
